Heath Declaration

I confirm that my child is not suffering from nor had any COVID-19 symptoms (cough, fever, sore throat, loss of smell or taste) in the last 7 days. My child has not knowingly been in contact with anyone with COVID-19 symptoms in the past 14 days. My child will not attend class should they or anyone in our household become ill as a result of illness, local lockdown, change in government advice, or advised by track and trace personnel. I do not know of any health condition which would put my child at greater risk of COVID-19.

I confirm that I have read and understand the conditons (including parental responsibilities) in Dani Grant School of DanceCOVID-19 Policy

I confirm that my child will adhere to social distancing throughout the class.
